My virtualmin says that there are softwares to update. Is it wise to always update? Do you guys always keep things updated?

If you care about security, then update it.

Thanks of course I care about security but do I need to look and make sure it will not break any thing on an existing server with a couple of sites?

For security reason, you should install the update but be sure to do a backup first before installing it. Some updates can cause issues to a website so that's why a backup is very important.
