Keyword density101: How can I adjust for better page results?

I have not been able to easily understand how to optimize for keywords, attached is how one page analysis tool sees my page, and other tools show similar. But, I am not sure how i wrote page text that gives emphasis to "hits homeless" <<----there aren't even any word combinations like that on the page. How do you SEO pros do it without looking like a bunch of header spam, especially when analyzing 2-3-4 word combinations, those have been even harder for me to optimize for?

Bold like this? (Will the page see this as a two word combinations?)

Underlined like this? (Will the page see this as a two word combinations?)

Italic words? (Will the page see this as a two word combinations?)

Header? (Yes, I know it will see all of these!)

"Quotes on text like this?" (Will the page see this as a five word combinations?)

How do you get the paragraph text to show better page density?

Writing all of this it makes me feel like I missed an SEO 101 class! I really have never seen how to do this efficiently.

I do not focus on keyword density at all. I just write (or have my writers write) about what needs to be written. I am ranking pages that have keyword densities of 12-14%. Sometimes, I don't even use a keyword at all on a page. When you are targeting 35 different keywords on one page, it's tough to include all 35 on the page.

Primary keyword goes in the title tag and either the exact same keyword or a very similar one goes in the H1 tag. That's about it. The rest of the content is just written without a major focus on specific keywords and instead focusing on the topic, or topics, at hand.
